Case Manager Sample Job Description
Case managers are healthcare professionals who help people with a large variety of issues. They assist both individuals and families in coping with complex physical or mental health conditions. Case managers are there to find the most effective treatment possible, so that their clients can live a long and happy life.
Summary
Assists and teaches clients how to identify their needs, access and maintain linkages to services such as DSS/SSI, Medicaid, housing, treatment and other services that support health and recovery.

Position Qualifications
Minimum Education & Experience
Case Manager II: Relevant MS and at least one year of relevant paid experience or bachelors degree in Social Work, Psychology, Counseling, or other related human service field or RN or CASAC and at least two years of relevant paid experience, or a minimum of three years relevant paid experience.
Case Manager I: High school diploma and less than three years relevant paid case management experience. Some relevant education preferred.
Licensure
• NYS motor vehicle license, safe driving record, and availability of personal car for work.
Other Skills/Knowledge/Experience
• Demonstrated knowledge of community resources including social security, public assistance and Medicaid systems.
• Demonstrated ability to relate effectively with culturally diverse persons who have chemical dependency and/or MICA problems.
• Effective written and oral communication and ability to work cooperatively with clients, staff and community professionals
